Here is a list of materials which are needed for review before placing your name with search committees:

1) A current ABPS profile or resume
2) Self-Disclosure and Release Form 
3) Covenant and Code of Ethics Form
4) Professional Church Leader Placement Form
5) GHC Agreement
6) A recent CD or DVD
